The Dental Practice Covered By This Notice
This Notice describes the privacy practices of Star Dental. “We” and “our” mean Star Dental. “You” and “your” mean our patient.
Our Responsibilities
We are required by law to maintain the privacy and security of your protected health information. We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information. We must follow the duties and privacy practices described in this notice and give you a copy of it. We will not use or share your information other than as described here unless you tell us we can in writing. If you give us permission, you may change your mind at any time by letting us know in writing.
How We May Use and Share Your Information
Treatment. We can use and share your information to treat you and coordinate your care with other providers.
Payment. We can use and share your information to bill and get payment from health plans and other payers.
Health Care Operations. We can use and share your information to run our practice, improve your care, and contact you when necessary.
With Business Associates. We may share information with vendors who help us run our practice. They must safeguard your information.
Public Health and Safety. We can share information to help with public health and safety issues, to prevent or reduce a serious threat to anyone’s health or safety, or for product recalls.
Law Enforcement and Legal Requests. We can share information with law enforcement, for health oversight activities, in response to a court or administrative order, or as otherwise required by law.
Coroners, Medical Examiners, and Funeral Directors. We can share information with these professionals as needed.
Workers’ Compensation. We can share information for workers’ compensation claims.
Other Uses and Disclosures. Other uses and disclosures not described in this notice will be made only with your written authorization.
Your Rights
Get an electronic or paper copy of your medical and billing records. Ask us how to do this.
Ask us to correct your records if you think they are incorrect or incomplete.
Request confidential communications. You can ask us to contact you in a specific way.
Ask us to limit what we use or share. We will consider your request. We are not required to agree to all requests.
Get a list of those with whom we have shared information for six years prior to your request, except for treatment, payment, and operations and certain other disclosures.
Get a copy of this notice. You can ask for a paper copy at any time, even if you agreed to get it electronically.
Choose someone to act for you. If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information.
File a complaint if you believe your privacy rights have been violated. You can complain to us using the contact information below. You can also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
Your Choices
You can tell us your choices about what we share. In these cases, you have both the right and choice to tell us to share information with your family, close friends, or others involved in your care, to share information in a disaster relief situation, or to include your information in a hospital directory if applicable. If you are not able to tell us your preference, for example if you are unconscious, we may go ahead and share your information if we believe it is in your best interest.
